How to Submit a Product
for Evaluation
The Carroll
Food Evaluation can
help you determine what your food intolerances
are, this is the first step to greatly improving
your health.
To learn more about this procedure,
please read the History
of Dr. Carroll.
NOTE: If you have
not been evaluated for YOUR food
intolerances using the Carroll Food Intolerance
Evaluation method, we will still be able to determine
what ingredients are in the sample. However, with
your specific evaluation results the food analysis can
be tailored to your particular requirements.
• SOH Members' ~ your fee is $10.95 per sample
to cover the costs of the evaluations.

• Windrose Naturopathic Clinic will
notify you of the analysis results once they are completed.
In order to achieve the most accurate
results when evaluating a food
for you and to determine if it is compatible for
your Carroll dietary
needs, we require the sample to be submitted in
this manner:
1) Click
here to download and print out the Food Submission
Form.
2) Fill out the ENTIRE form.
3) The sample must be submitted in
a sterilized, clean GLASS JAR.
(The small glass bottles that supplements are often
sold in work well.)
4) The sample must be at least 1
Tablespoon in volume.
5) Pack
Sample Jar in box with care.
The following must be submitted
with your sample, otherwise we will not be able to complete the
analysis for you.

6) Mail sample packet to:
